WAIVER and PROCEDURES I will be responsible for fostering a dog/puppy for Best Friend Pet Adoption (BFPA). I will be responsible for the day-to-day care and safety of this animal and will comply with the procedures listed below. BFPA will be responsible for all vet work and expense, including monthly heart worm and flea/tick preventative. Food will be supplied if the foster cannot provide food. BFPA will also supply bowls, leashes, collars and a BFPA ID Tag for your foster dog's collar. If you have a crate and do not need one of ours, please let us know. As a foster parent, you will be responsible for getting the foster dog to and from vet appointments, adoptathons, and other events unless prior arrangements have been made with other volunteers. An email will be sent out by a BFPA representative to let all fosters know of upcoming events. All vet appointments must be made by a BFPA Dog Medical Coordinator, no exceptions. Dog fosters will be given all necessary phone numbers and email addresses for the person responsible for setting up vet work for your foster dog. Feel free to contact a Dog Medical Coordinator if you have any questions or concerns. Be aware that you will need to take all copies of vet work with you each time you have a vet appointment for your foster dog. BFPA makes no guarantees to any animal’s personality or behavior. BFPA cannot be held liable for any bodily or property damage caused by a BFPA foster dog. We will always do whatever we can in the rare event that an unfortunate situation presents itself; however, we are a non-profit organization with limited financial resources. Please be aware that reimbursement for any supplies bought for your foster dog must be pre-approved. Items such as food, toys, and crates are donated and we may have in our storage units what you need. If you are pre-approved for reimbursement, please submit a receipt with the items you are claiming circled or starred to our PO Box address (address on the homepage of our Web site) and include your mailing address. We will issue a check within 14 days. We thank you for your interest in fostering a dog for BFPA. There are no more important volunteers to our rescue group than those who open their homes to our animals. Please know that you are greatly appreciated. Signed (type your name)*
