Fosters are expected to provide basic training and enrichment for foster animals, work with trainers (if necessary) and attend training events that the rescue makes available to fosters. The rescue is available for guidance throughout. Are you willing to take on this responsibility?

Part of the foster role is transporting your foster animal to vet appointments and events. Events are critical opportunities for your foster animal to be seen by potential adopters. Fosters are expected to stay at events to help promote their animal. Do you accept this responsibility?

I understand that animals will be vet checked but may show signs of illness while in my care. Should any symptoms arise, I am to contact Forget Me Not Animal Rescue (FMNAR) promptly. In the event of an emergency, I will contact FMNAR and have the animal taken to Porter Animal Hospital or another specified vet, for treatment. FMNAR will not reimburse out of pocket expenses should you choose to take the animal to your own vet, without permission. I understand that FMNAR is NOT responsible for my family's health or pet's health, or any injury that may occur as a result of exposure to the foster animal. I also release FMNAR from liability for any damage done to my property or belongings. I understand that the animal(s) in my care may require daily medications or treatments, which I will be responsible for giving, per care instructions. I understand that I must make every attempt to get my foster animal to scheduled vet visits and pick up any necessary medicines in a timely manner. I understand that transporting my foster pet to vet visits is part of my responsibility and mileage is tax deductible. I understand that FMNAR will provide necessary supplies for the foster animal but I am responsible for ensuring that the animal receives adequate daily food and fresh water. Supplies include foods, medicines, crate etc. I understand that I may choose to purchase toys, treats etc. for my foster, at my own expense. These expenses can be claimed on your taxes as an eligible deduction. I understand that introducing foster animals to my own pets has inherent risks. It is recommended that foster animals remain separated from personal pets. However, if foster pets and resident pets are to be introduced, it MUST be with the guidance of FMNAR. I agree to accept all responsibility from the outcome of their interactions. FMNAR will provide guidance and REQUIRES decompression time of a MINIMUM of 2 weeks for all foster pets. I understand that the foster remains the property of FMNAR who will be responsible for making all decisions regarding the foster animal. I will respect and adhere to those decisions. I understand that foster animals may not be rehomed or placed in a different foster home without permission from the rescue President/Director. I agree to bring my foster to select adoption events to meet potential adopters or work to schedule a meet win another location, such as my home. If I am unable to transport the foster, I will contact FMNAR in order to schedule alternate transportation, via a qualified volunteer. I understand that foster families are intended to keep foster animals until they are adopted (unless I am a short term stand in foster only) and that it is impossible to place a time frame on how long I will have this animals. If I am no longer available to foster, I will give FMNAR as much notice as possible. I recognize that FMNAR does not have ready, open foster homes and will not place an animal in an unsuitable situation, as a result of your decision. As a result, I may have to wait until another foster can be found. FMNAR will do everything possible to re-place the animal as quickly as possible. If FMNAR requests the return of a foster animal, I must return the animal within 48 hours. I must also return all property purchased for the foster animal at the time of return or once my foster animal is adopted.
