PLEASE NOTE THAT THIS FORM IS NOT AN OFFICIAL RCHS APPLICATION TO ADOPT AN ANIMAL. 

If you have already decided which pet you would like to adopt and want to move ahead with the adoption process, then download our Pet Adoption Application Form (MS Word doc), fill it out, and email it back to us.

Please keep in mind that RCHS is staffed by volunteers, so it could take us a few days to get back to you. If you haven't heard from us within one week, please give us a call at (304) 636-7844. 

And if you haven't had a chance to meet your chosen pet yet, feel free to give us a call to arrange a meeting, or stop by the shelter during normal business hours.
