This agreement is made and hereby entered into by and between Saving Great Animals (hereinafter referred to as “SGA”) and Foster (hereinafter referred to as “Foster”) on the date below.  A foster home is defined as temporary housing for any SGA dog while it is being screened and approved for permanent adoption. All fosters must be 21 years or more of age.

Foster understands and agrees to the following terms and conditions:

• I agree to care and love any foster dog given to me by SGA as if it were my own.
• I agree to provide the dog with exercise, basic obedience training and house training.
• I agree to bring the dog to at least one SGA adoption event each month. 

MEDICAL CARE

• I understand that all dogs placed in foster care need to be spayed/neutered and vaccinated. If I receive an unfixed dog, SGA may request that I schedule an appointment with a designated SGA veterinarian and assist in transporting the dog to and from the appointment. If I cannot arrange to do the transport myself, I will call my SGA representative and assist in any way I can.
• I understand that my foster dog may need veterinary care for medical issues other than spay/neuter or vaccinations and agree to take my foster dog to an SGA approved vet at SGA's request.
• I understand that if a dog was recently spayed/neutered or had surgery of any kind, I need to protect the incision by keeping it dry and clean, and by having the dog wear a cone if necessary.
• I understand that, in some cases, shelter dogs can develop medical issues such as kennel cough after arrival with SGA. I understand that rescue dogs may come with unforeseen medical issues and that measures are taken to place healthy animals in foster homes only to the extent that this is possible.  

EXPENSES COVERED BY SGA

• I understand that all basic medical expenses for each foster dog that comes into my care will be covered by SGA,  provided that an authorized SGA representative has approved the medical expenses, and the care is from a SGA designated veterinarian.
• I understand that if I choose to go to a vet of my choosing without authorization, I will be responsible for most of the vet bill. A list of approved veterinarians is available through SGA. 
• I understand that SGA will provide a tag, and if needed a collar, harness and leash for each foster dog that comes into my care. SGA appreciates willingness to donate toward the dog’s general expenses, like food, treats, etc., and SGA will offer donated items, as available, for use by foster homes (Such general expenses, if paid for by a foster home and not by SGA, may be tax deductible.) Crates can be lent to foster homes and returned to SGA when a foster dog is adopted.

DOG DESCRIPTION AND PHOTO ON WEBSITE

• I agree to assist in getting a photo of each foster dog that comes into my care on the SGA website. I agree to either take a photo and write a short description myself, and submit it to SGA for review and assistance, or ask for assistance in taking the photo and writing the description of each foster dog in my care.

ADOPTION/TRANSFER

• I understand that any potential adopter MUST be screened, interviewed and approved by an authorized SGA representative. I understand that my input will be appreciated and a part of the process, and that the final decision to place or not to place any particular dog with any particular potential adopter will be made by an authorized SGA representative.
• I agree to let SGA know if prospective adopters contact me directly by phone or email, and to send copies of any email inquiries to my SGA representative. All inquiries and applications for a foster dog MUST come through SGA. No meet and greets can take place without an application on file.

PROTOCOL WITH CHILDREN

I understand that the following protocol should be followed regarding children in the house:

• Do not under any under any circumstances leave a child unattended with any dog.
• Do not leave bones around the house when children are present and be cautious and observant when children are eating and the dog is in the room.
• Children under the age of 13 will not walk the dog unless supervised by an adult
• Whenever a child is in the house with a dog, the child is at risk of being accidentally knocked over, scratched or bitten etc.
• I certify that I do not have children under 6 years old in the home and understands that having a child under 6 years old in the home disqualifies me from fostering for SGA.
• Should you have ANY concerns or feel uncomfortable with a situation between the dog and a child contact an SGA representative. We will either take the dog out of your home immediately or have a trainer come and meet with you, depending on the severity of the situation.

CATS AND DOGS CO-EXISTING

• I understand that even if this animal has been housed with cats in its foster home that SGA cannot guarantee the behavior with a new cat(s) will be the same. Please use caution when introducing a new dog to existing cats in the household and do the introduction slowly over a period of a few weeks, keeping the dog on leash. If you need assistance, please do not hesitate to contact an SGA representative.

MULTIPLE DOG HOUSEHOLDS

I understand that the following protocol should be followed when bringing additional dogs into the house:

• Do not leave multiple dogs at home in the same room unattended.
• Do not feed the dogs in the same room and don’t leave toys and treats laying around the house.
• SGA can put you in touch with either a senior volunteer or a behaviorist to assist you with a dog to dog introduction if necessary.

LEASH RULES/DOG PARK RULES:

• I understand that the SGA ID tag must be worn on the dog’s collar at all times, because this is its link back to SGA. If the dog loses its tag, I agree to immediately contact an SGA representative for a new one.
• I agree to keep each foster dog that comes into my care on two leashes when outside my home or my own securely fenced yard. 
• I understand and agree that NO foster dog that comes into my care will be allowed to run or play off- leash in a dog park or other open public area.
• I understand and agree that I am responsible for the safety of each foster dog that comes into my care, for the safety of other dogs and people the dog comes into contact with.
• I agree to immediately notify my SGA representative in the event of a missing or injured foster dog. I understand that if my foster dog goes missing in my care, I will be responsible for any expenses related to recovering the dog. 

ACTIVITIES RELATED TO DOGS:

• I understand that various activities involving dogs and/or puppies can be considered hazardous in nature and involve the possibility of being bitten, scratched, jumped on, dragged or knocked over. The possibility of serious accidents and/or injury can occur during my time as a foster. Knowing the risks involved nevertheless I voluntarily apply to participate. 
• I understand that if I sustain a dog-related injury such as a bite or scratch, I will follow the appropriate cleaning and wound care instructions and will seek medical attention as required.
• I understand the potential safety risks of fostering dogs with SGA and understand that I am accountable for the actions of the other people in my household, including visitors. I understand it is my responsibility to ensure SGA’s rules and protocols are followed by all household members, including visitors. 

• I understand that if I decide to adopt a foster dog in my care, I must go through the adoption process, including paying the requested donation.  
• I understand that if I need to return a dog to SGA’s care, for any reason, enough forewarning is necessary for SGA to locate a new foster home placement for the dog, and that this process may take 2 days or more, depending on the dog and the number of other dogs in foster care.
• I understand and agree that SGA does not approve of punitive training methods and agree to never use a shock collar, prong collar or choke collar for my foster dog. 
• I understand that SGA retains the right to remove a foster dog from my care at any time.
• I agree to immediately report any bite incidents or behavioral concerns to SGA. 

RELEASE OF LIABILITY

In consideration of my desire to foster with Saving Great Animals, I hereby assume all responsibility for any and all risk of property damage or bodily injury that I may sustain while participating in any voluntary relief effort or other activity of any nature, including the use of equipment and facilities and the handling of animals.

I currently have no known mental or physical condition that would impair my capability for full participation as intended or expected of me. Further, I have carefully read the foregoing release and indemnification and understand the contents thereof and sign this release as my own, free act.

Saving Great Animals is a non-profit animal rescue . In this capacity SGA rescues, socializes, trains, rehabilitates and finds new homes for dogs, some of whom have been neglected, are victims of abuse or are under-socialized. In some cases, the history of the dog may have lead to special behavioral issues and requires cautious interaction.

This document provides for the release of liability of Saving Great Animals (SGA) and its volunteers on account of the actions of any dog of any breed, which has been released into the ownership of SGA and placed into the care of a Foster Care Provider (“any fostered dog”).
I, as the Foster Care Provider, understand that SGA and its volunteers cannot guarantee actions or disposition of any fostered dog placed into Foster Care with me. I understand that although reasonable effort has been made to assess the disposition of each dog released to SGA beforehand, being living animals, they may act unpredictably in any given situation, and that I must take necessary precautions to prevent accidents and guard against unpredicted actions by any fostered dog in my care.
