Everyone should fill out the first page of the volunteer form and then there are optional pages for special events volunteer, transport volunteer, and fostering.
Volunteers need to be 16 year of age or older to work with the dogs at the shelter. Volunteers must also be able to work without supervision. Volunteers under 16 may be accepted on a case by case basis, and may require parental supervision while at the shelter. We are sorry, but children under 12 are not allowed to be volunteers.

If the dog should appear sick, become injured, become lost, or bite me or another person or animal while in my care, I will notify the animal caretaker immediately.

I understand that I am taking responsibility for this dog's care and actions while I am fostering.
I will follow all instructions regarding the dog's medical care giving any prescribed medications as well as monthly heartworm/flea/tick preventative which Bichon and Little Buddies can provide.
While fostering this dog, I will not foster a dog for another rescue.
When outside, the dog will be kept on a leash, line, in a securely fenced area, or in a kennel/crate.
I will keep a tag on the dog that includes my phone number as well as the rescue tag in case the dog becomes lost.
I will not put this dog in a situation that could be dangerous for him/her.
Dogs being fostered for Bichon and Little Buddies are not to be taken to dog parks or similar areas.
If this dog should become sick, injured, lost, or bites me, or another person or animal, I will notify Patti Muraczewski 414-750-0152 or Tammie Riepe 414-422-9222 immediately.
I will arrange for the dog to be met by prospective adopters, either by appointment or during open hours at the shelter facility.
I understand that if I decide I would like to adopt this dog, I must notify Bichon and Little Buddies and complete an adoption application prior to another applicant being approved to meet the dog.
I understand violating any of this agreement will end my ability to foster with Bichon and Little Buddies Rescue and the dog will be returned promptly to the rescue.
