PJ’s Rescue makes every effort to adopt to you a dog in good health, but we are not a medical organization. Under no circumstances will PJ’s Rescue reimburse the adopter for medical expenses incurred after adoption. Therefore, as the potential adopter, I understand that PJ’s rescue makes no warranty as to the health and temperament of the animal I am adopting. If a veterinarian finds any illness or defect in this animal, I am responsible for veterinary fees to treat the animal. PJ’s Rescue will not be held responsible for any vicious behavior or damage the animal may cause after adoption.
I agree that I am adopting the above-described animal solely as a pet for myself and/or my immediate family. I agree that I will not sell, give away, or otherwise dispose of said animal to any person(s) or other entity for any reason without the prior approval of PJ’s Rescue. I am aware that there will be NO refunds if the adoption does not work and I break this agreement. If at a later date I am unable or unwilling to keep this animal I agree to contact PJ’s Rescue and make arrangements to discuss veterinarian care, training, or rehoming options for this animal.
I agree to the humane care of this animal and to provide it with clean and appropriate shelter, food, water, exercise, companionship, and veterinary care. I agree to keep my animal up-to-date on vaccinations to prevent disease, as well as, heartworm preventative as per my veterinarian’s recommendations. I am aware that PJ’s Rescue does not adopt any animal to any individual(s) intending to keep it as solely an outdoor animal.
I understand that PJ’s Rescue adopts animals as companion animals only. I agree that this animal shall never be used for medical research, dog fighting, or any purpose outside of being a companion animal. I will not give/sell/lease this dog to any other party who intends to use it as such. I am aware that legal action can be taken against any person falsely agreeing to this contract and not following through with its content.
I will abide by the local leash laws and keep this dog licensed as required by the town or city that I reside. I will keep ID tags on my dog at all times. I will keep PJ’s Rescue as a secondary contact for all microchipping information and I will immediately notify authorities if my animal becomes lost or is stolen.
I understand that PJ’s Rescue has the right to reclaim this animal if any of the above conditions are not met. I am aware that PJ’s Rescue may make follow-up visits at a mutually convenient time to ascertain that the above conditions are satisfactorily met.
If the animal that I am adopting has completed the prison training program, I understand that it may be a requirement for me to follow up and complete the additional free training through R&R training in Carlton, MN.
Permanent ownership of this animal is contingent upon compliance with this Alter Agreement between PJ’s Rescue and myself, noted as the ‘Adopter’. In consideration of an adoption fee, PJ’s Rescue will temporarily transferred all rights, privileges, and responsibilities associated with the animal described in the Adoption Agreement to the 'Adopter'.
Due to current COVID related vetting delays, the 'Adopter' acknowledges that as of the date of this Agreement, the animal MAY NOT be spayed or neutered. If unaltered, the Adopter will do their due diligence to keep the animal away from other unaltered animals to prevent future litters. If the 'Adopter' choses to alter the dog through their own veterinarian, the 'Adopter' agrees to have the animal spayed or neutered within 90 days of adoption. PJ’s Rescue reserves all rights to revoke this timeline when vetting resumes normal timelines.
If the 'Adopter' has chosen to have PJ’s Rescue provide the spay or neuter service, the 'Adopter' agrees to make all efforts to have the dog to the noted veterinarian (as provided by the PJ's vetting team) as scheduled. You are required to purchase a cone (you can purchase at veterinary office) and follow the checkout guidelines you receive from staff for a smooth recovery. If you have any questions after surgery, please email the vetting team at vetting@pjsrescue.com Please DO NOT call the veterinary clinic who completed the services. If the 'Adopter' cannot make this appointment, they agree to communicate with the PJ’s Rescue team for an alternative direction. The 'Adopter' agrees that if the dog misses their appointment and no communication was had, they will be responsible for the $150 no-show fee.
If the 'Adopter' fails to provide timely proof of spay or neuter within the time set forth above, or fails to attend the scheduled alter appointment, this Agreement will be enforced and the appropriate legal authority will be notified. In addition, all rights and ownership of the animal shall be automatically terminated and transferred back to PJ’s Rescue and PJ’s Rescue will immediately assume possession of the animal. The 'Adopter' shall pay all reasonable attorney’s fees incurred in attempts to repossess the animal, as well as $150.00 in damages. If proof of spay/neuter is provided after the time limits set forth above, the deposit shall remain forfeited and the ownership of the animal shall be determined on a case-by-case basis.
PJ’s Rescue may allow extensions of the time periods upon receipt of a certificate from a veterinarian licensed to practice veterinary medicine in the United States that certifies that the animal has a medical condition which prevents him or her from being spayed/neutered, or that it would otherwise be detrimental to the health of the dog/cat to be spayed/neutered. In such a case, the animal shall be spayed or neutered when the animal is healthy enough, as certified by a veterinarian licensed to practice veterinary medicine in the United States. The animal shall be spayed or neutered within ninety (90) business days of that certification, or the conditions set forth above will be enacted.
If spay or neuter procedure is performed through the adopter's own vet, proof of (a spay/neuter certificate or receipt) must be sent in the form of a scan or photo to vetting@pjsrescue.com.
