I certify that all information given is correct, and I understand it will be verified.
I understand that if a crate (or any other item, such as a play pin, leashes, etc.), is borrowed from Take Paws Rescue, it is to be used for the dog(s) I am fostering and is to be returned to the rescue if at any point I stop fostering, take a break between foster dogs, or at the request of Take Paws Rescue. If the crate (or any borrowed item) is broken/damaged for reasons such as, but not limited to, neglect or improper use, I will be responsible for replacing the item with the same or an agreed upon replacement.
I understand that if I commit to fostering a dog, there are usually adjustment periods. If the dog turns out to not be a good, workable fit, I must allow the rescue time to find another suitable foster for the dog before trying to help save another.
I understand my responsibility to my foster to provide them with loving, nourishing environments to help them reach their full potential.
I understand that I will not be allowed to foster a dog until I have read and understand the Foster Guide that will be provided to me upon my approval to foster:
If the dog I foster is on medication I will continue the medication as directed.
I will not let the dog out loose by itself.
If during the time I am fostering the dog, he/she requires medical attention, I will bring it to the vet and contact my foster coordinator.
I understand that in the case that I evacuate my home for a storm or other type of situation, the dog must evacuate with me.
I understand the foster dog is the property of Take Paws Rescue and will not sell, trade or dispose of the dog.
I understand the importance of my own animals being up to date on all standard vaccinations.
I understand that anyone interested in adopting my foster dog (including myself) must go through the standard adoption process for approval and final decision on placement of the dog is made by Take Paws Rescue.
I understand that while Take Paws Rescue takes reasonable care to screen dogs for foster care placement, it makes no guarantee relating to the dog’s health, behavior or actions.
I understand that I receive the foster care dog at my own risk and can reject or return any dogs for which Take Paws Rescue has asked me to provide care.
I indemnify and hold Take Paws Rescue free and harmless from all liability arising out of any and all claims, demands, losses, damages, action, judgement of every kind and description which may occur to or be suffered by me, members of my household, my own animals or any third parties by reason of activities arising out of this agreement.
I release Take Paws Rescue from responsibility for any diseases that may be contracted by my resident animal(s) from the foster dog.
I have reviewed the terms and condition set out herein. By checking this box I indicate that I understand and agree to abide by the same and hereby execute this contract.
